What is the Home Visit Nursing Service?

For those who want to be cared for in their own home, our nursing staff with knowledge of the individual’s condition will visit his/her home, ensuring a safe and secure everyday life

To use this service:

  • -To use the Home Visit Nursing Service, a “Homon Kango Shijisho (Home Visit Nursing Service statement of direction)” by your primary care doctor will be required.
  • -As a general rule, using Long Term Care Insurance is priority, but this depends on your disease and National Health Insurance conditions may be applied under the direction of your primary care doctor.
  • – We create a plan discussing with our service user’s primary care doctor and care manager, and we design a suitable care package with the style of service required including how often visitation by care providers is required, for each individual patient.

  • -Concern about recuperation at home after discharge from hospital.
  • -Don’t know how to be nursed at home because it’s your first time.
  • -Using day service, but would also like to do rehabilitation at home as well.
  • -Don’t have a confidence to look after your parents at home, as they often forget things.
  • -Having trouble about dealing with medicine, such as forgetting to take medicine, or confusing the types of medicine you are taking.

Service’s details

Home Visit Nursing Service supplies services as follows.

-Support for everyday life

We support meals and toilet, cleanliness, medication management, and assistance and advice in a variety of everyday life activities.

-Observation of symptoms

We check our service user’s every day physical and mental health condition, such as body temperature, pulse, blood pressure, breathing, and early detection of abnormalities in health conditions.

-Management of medical treatment and medical equipment

Following primary care doctor’s instructions, we preform medical treatment (suction, infusion or injection, stoma care and the prevention and treatment of bed sores), and management of home oxygen and ventilator.

-Rehabilitation

Training activities in daily life (eating, toilet, walking, etc.), avoid being bedridden and helping to return to social life.

-Care, support and counseling the family.

We support not only our service users but their family too, with counseling and advice about home care support and the use of social resources.

-Dementia care

Support and consultation for dementia care.

-Terminal care

For our service users to have their last moments in comfort, relaxation and peace, we support daily life activities, advice and help managing end-of -life care systems until the end.
